Cash Benefits (Cash Reimbursement)

This page is for the following target audience.

  • Individuals who are or have been eligible for the Medical Expenses Assistance Program for Infants and Young Children, Compulsory Education Students, and High School Students (Maru-Nyu, Maru-Ko, Maru-Sei)
  • Those who visited medical institutions outside Tokyo and could not use their medical certificate
  • Those who have received medical treatment at medical institutions before the issuance of the medical certificate due to procedures such as birth or relocation.
  • Those who have visited medical institutions in Tokyo that do not have a medical expense subsidy system
  • Individuals who have incurred costs for therapeutic devices (such as glasses for low vision and assistive devices)
  • Those who have had their medical expenses fully covered (100% coverage)
  • Those who are enrolled in the National Health Insurance outside Tokyo
  • Those who have borne the out-of-pocket expenses for other medical expense assistance programs
  • Those who were unable to present their medical certificate to medical institutions for unavoidable reasons

Notice

  • Items not covered by health insurance (Health Checkup, Vaccination, treatments not covered by insurance, etc.) are excluded.
  • If your child becomes ill or injured at school, kindergarten, nursery school, etc., they may be eligible for benefits under the Japan Sports Promotion Center Law. Please check with the school in advance.
  • If you are injured due to the actions of others (such as in a traffic accident), separate procedures will be required.

About Grants

Within the qualification period of the medical certificate, the amount of co-payment for insured medical treatment paid at medical institutions, etc. will be refunded as a subsidy.
Please fill out the Application Form and submit it along with the required documents.

When receiving medical treatment outside the city and presenting only the qualification confirmation document, etc., before the issuance of the medical certificate (when bearing 20% or 30% of the Medical Expenses)

    Note: Please apply separately for inpatient and outpatient portions.
  2. Receipt (Original)
  3. Maru Milk, Child, and Blue Medical Certificate (Original required for in-person procedures. Copy required for submissions by mail.)

If you want to change the transfer account

  • Cash card or bankbook (original when applying at the counter. Copy when submitting by mail.)
    Please bring your passbook if you are using Japan Post Bank.

Those who have medical certificates from other systems

  • Certificate of Eligibility for Maximum Amount, Children's Chronic Medical Care Certificate, etc.

When the cost of glasses and assistive devices for low vision is covered

  1. Application Form for Medical Expense Subsidy (You can download it from the link below or it is also available at the counter)
  2. Receipt (original is preferred. A copy is acceptable if it has been submitted to the insurer.)
  3. Maru Milk, Child, and Blue Medical Certificate (Original required for in-person procedures. Copy required for submissions by mail.)
  4. Notification of Benefit Decision from Health Insurance Association (Original)
    Note: After the subsidy procedure with the Health Insurance Association, the subsidy procedure with City Hall will follow.
  5. Physician's instruction or diagnosis document (original copy is required in principle. If submitted to the insurer, a copy is acceptable.)

When making payments at medical institutions, if neither the qualification confirmation document nor the Health Insurance Card was used, please submit the following documents to the city office after the health insurance union's subsidy procedures.

In the case of high-cost medical expenses

High-Cost Medical Expenses refer to a system where if the same insured person (or dependent) incurs medical expenses exceeding a certain amount within the same month for the same medical department (amounts are combined for the same household), medical expenses are subsidized later by the health insurance association in addition to the 30% copayment.
The Child Medical Expenses Subsidy System provides assistance for the difference after deducting the portion covered by the Health Insurance Association from the insurance-covered medical expenses. (In the case of Maruko and Maru-Ao, 200 yen per outpatient visit is excluded.) If you qualify for high-cost medical care benefits, please apply for the benefits through your Health Insurance Association. For application procedures, please contact your Health Insurance Association.
After the Health Insurance Association Subsidy Procedure, please submit the following documents to the city office and apply for the difference in Medical Expenses.

Points to Note When Submitting by Mail
The acceptance date by mail will be the date the application form or notification arrives at City Hall.
If the arrival date at City Hall is delayed due to postal accidents, the effective date of eligibility for benefits may be delayed (the month you start receiving allowances or the start of the validity period of the medical certificate may be delayed). To ensure that the Application Form or notification reliably reaches City Hall, please send it using methods such as specified recorded mail or simplified registered mail.
