Notification Regarding Co-resident Children
In accordance with Article 30, Paragraph 1 of the Child Welfare Act, those concerned must submit a Notification to the prefectural governor through the mayor.
Eligible Individuals
A person who has housed a child who is not within the fourth degree of kinship, separated from the person exercising parental authority or the minor guardian, in their own household (including single-person households) with the intention of cohabiting for more than 3 months (1 month for infants), or who has continuously cohabited for 2 months or more (20 days or more for infants)

Required Items for Notification
- Above Notification Form
- Copy of Certificate of Residence
- Documents that can verify the identity of the notifier (driver's license, My Number Card, photo Basic Resident Registration Card, etc.)
