Fiscal Year 7 After-School Children's Program Registration and Snack Application

About User Registration

You can register for use and apply for snacks through the application form.
If you have siblings, each person must apply individually.
If it is within the application period, you can use it 24 hours a day from your smartphone or computer.
When using the service, please be sure to check the "After-School Children's Program Guide" which contains detailed information before registering.

Before Registration

When applying, we recommend creating an account on the LoGo form using the link above.
By creating an account, Parent or Guardian information will be automatically reflected in the application form, allowing you to skip entering your name, email address, and other details.
You can also check your application details from My Page after applying.

Registration Period

If the application registration is completed by the 18th of each month, it can be used from the 1st of the following month.

• Note: Applications cannot be made from 12:00 AM to 12:00 PM on the 19th (if it falls on a Saturday, Sunday, or holiday, applications cannot be made until 12:00 PM on the next business day).
• Note: The usage period is from the 1st of the month of commencement until March 31, 2028. Even if you were registered in the previous year, registration procedures are required.

Usage Fees and Insurance Premiums

Usage is free of charge. However, insurance enrollment is required for registration. The insurance fee is 624 yen (annual amount, including administrative service fee).
Even if you register for use in the middle of the fiscal year, the insurance premium will not change. Also, if you stop using it in the middle of the fiscal year, the insurance premium cannot be refunded.
Note: Once the registration is completed, refunds cannot be made, so please carefully consider before applying.

Payment Method

Registration is done through payment via PayPay or credit card.

Other Notes

• Due to system maintenance of the site, the application process may not be available, so please apply with plenty of time before the deadline.
• A completion notification will be sent to the specified email address after registration is complete. The completion notification will be sent from no-reply@logoform.jp. Depending on the email address you receive it at, it may end up in your spam folder, so please make sure to set it up in advance to ensure you can receive it.
• Please write the reception number listed in the registration completion email on the 'First Use Card' and bring it to the After-School Children's Program. We will provide you with an attendance card during your first use.

About Snacks

Application Period

Snacks can be used in the following month for the number of meals applied for between the 1st and 20th of each month.
Note: Once the application procedure for snacks is completed, cancellations are not allowed. Please make sure to coordinate the desired quantity with the child in advance before applying.

Usage Fees

The snack costs 110 yen per serving (including administrative handling fees).
Note: Please apply based on the number of meals for the following month.

Payment Method

The snack fee can be paid using PayPay or a credit card.

Other Notes

• Snacks will be provided on days marked with a circle in the snack section of the attendance card.
• The snack application site includes the details and ingredients of the snacks that will be provided the following month. If the child has allergies, please be sure to check, and if there are items that contain allergens, please refrain from ordering.
• You cannot specify an item when applying for the provided items. You also cannot specify the snacks you wish to use.
• Please be aware that if there are more users than usual due to Parent or Guardian meetings, we will not be able to provide a place for snacks.

Change of Registration Details and Departure

If there are any changes to the registration details (address, school, etc.), please inform each After-School Children's Program when leaving during the school year for reasons such as entering the After-School Childcare Program.
