About the Application Form for Certification of Medical Expenses Support for Outpatient Mental Health Care

There is the Household Income Survey and Consent Form, but wouldn't it be sufficient to stamp the Application Form and have the applicant sign it?
I used to live in Fuchu City, and this was the method used there.
It's a waste of paper, especially since we have the My Number Card, and on top of that, there is only one copier on the floor.
Even though I informed the staff two years ago, no improvements have been made. Although the example of Fuchu City was also discussed at the counter, in the end, it is the same if the information is checked via the terminal.
