Request for Cooperation in the "Home Care Situation Survey"

In Inagi City, we will conduct a "Home Care Status Survey" as basic data for the formulation of the Inagi City Community-based Integrated Care Plan (the 5th Senior Welfare Plan of Inagi City and the 10th Long-Term Care Insurance Business Plan of Inagi City). Accordingly, along with the certification survey related to certification renewal and classification application, the certification surveyors will inquire about your daily living conditions, so we kindly ask all citizens to cooperate with the survey.

Survey Overview

Survey Period: From October 20, 2025 to the end of June 2026 (planned)

Eligible persons: Those who are certified as requiring support or long-term care and living at home, who will undergo certification surveys related to renewal applications or category change applications, and their family members providing care.

Survey response method: The certified surveyor will fill out the questionnaire or conduct an interview.
