For nursing care services and nursing care prevention and living support service projects, it is necessary for users to bear a certain percentage of the costs.
The Long-Term Care Insurance Burden Ratio Certificate informs users of their burden ratio when utilizing nursing care services, nursing care prevention, and living support service projects.

Please present it when using the service

Please keep the burden ratio certificate together with the Long-Term Care Insurance beneficiary certificate, and be sure to present both documents together to the service providers or facilities when using care services.

We will send the Long-Term Care Insurance burden ratio certificate by mail

For those certified as requiring long-term care or support, and those receiving nursing care prevention and living support service projects, the burden ratio certificate is sent sequentially in mid-July. The burden ratio is determined annually, so please be sure to check it.
The applicable period for FY2026 is one year from August 1, 2026 to July 31, 2027.

Determination of Burden Ratio

The burden ratio varies from 10% to 30% depending on income status (see Figure 1).

(Note) Individuals aged 40 to 64 are responsible for 10% of the costs.
  • Note (1) Total income amount refers to the income amount after deducting public Pension and other deductions, salary income deductions, and necessary expenses, before applying basic deductions and personal deductions. It is also calculated after deducting the special deductions related to long-term capital gains and short-term capital gains.
  • Note (2) The total income amount other than that refers to the income amount obtained by excluding the miscellaneous income from the Pension from the total income amount in (1).

Reissuance of Burden Ratio Certificate

Reissuance is available at the Senior Welfare Division, Inagi City Hall (2nd floor counter). (Free of charge)
Please submit the Application Form for the reissuance of the Long-Term Care Insurance Health Insurance Card.
