Request for Cooperation in the "Preventive Care and Daily Living Area Needs Survey"

Survey Overview

In Inagi City, in the process of formulating the Inagi City Community-Based Integrated Care Plan (Inagi City Senior Welfare Plan (5th term) and Inagi City Long-Term Care Insurance Business Plan (10th term)), we will conduct the "Preventive Care and Daily Life Area Needs Survey" to more accurately grasp the issues of the community and Older Adults.
When the questionnaire arrives, please cooperate with the survey.

Survey Period

From early January 2026 to Friday, January 23, 2026 (planned)

Eligible Persons

A total of 4,700 people randomly selected as of December 1, 2025, including those residing in the city who are eligible for the Comprehensive Care Prevention and Daily Life Support Project, those certified for care levels Support Required 1 to Support Required 2, and those aged 65 or older without care certification
