When you want to change the mailing address for documents related to Late-Stage Elderly Medical Care

Documents related to Late-Stage Elderly Medical Care are generally sent to the individual's registered address. However, if there are special circumstances such as the individual being unable to manage the documents themselves or entering a facility, you can submit a "Request for Change of Delivery Address for Late-Stage Elderly Medical Care Documents" to change the delivery address to another location (such as a family member or legal guardian).

Note: This application is for changing the delivery address of materials related to Late-Stage Elderly Medical Care insurance. Changes to delivery addresses for Long-Term Care Insurance and other services must be made separately.

About Applications by Mail and at City Hall

If you wish to apply by mail or at City Hall, please download and print the form below, fill in the required information, and enclose copies of the necessary documents listed below before mailing them or bringing them to City Hall. After receipt, the change of delivery address will be registered following a review. If there are any deficiencies or unclear points in the application, we may contact you by email or phone.

Insured Person's Identification Documents

 Note 1: One type of photo ID (driver's license, My Number Card, etc.) is sufficient.

 Note 2: Please prepare two types of documents if you do not have a photo ID (such as Late-Stage Elderly Medical Care Qualification Certificate, Long-Term Care Insurance Card, Pension Book, etc.).

 Note 3: When mailing a copy of your My Number Card, please attach a copy of the side with the photo, and do not attach a copy of the side with the My Number.
