Maternal and Child Health Handbook

At the Parent and Child Comprehensive Support Center, we provide the 'Maternal and Child Health Handbook' and the 'Mother and Child Health Bag' to those who are pregnant.

Once you are diagnosed with pregnancy, please submit the pregnancy notification form according to the instructions of the diagnosing physician and receive the Maternal and Child Health Handbook and the Mother and Child Health Bag.
The Maternal and Child Health Handbook allows you to record the health status of mothers and children related to pregnancy, childbirth, and childcare, which is helpful for health management. The 'Mother and Child Health Bag' contains not only the health checkup receipt for pregnant women but also information necessary for childbirth and childcare.

Eligibility

Pregnant women registered as residents in Inagi City
Note 1: A letter of authorization is required when a representative submits the notification on behalf of someone other than a pregnant woman.

Place of Notification

Parent and Child Comprehensive Support Center
Note 1: (1) Hirao Branch Office, (2) Wakabadai Branch Office, (3) City Hall Citizen Affairs Division (only on holidays) are also available for acceptance.
Note 2: When visiting the counter, please check the reception hours of each location before coming.

Belongings

Bring one of the following (for the person submitting the notification)

  1. One document with a photo issued by a public office (such as a personal identification card, driver's license, passport, residence card, etc.)
  2. Two documents without a photo issued by a government office (such as various health insurance qualification certificates, Pension handbook, or Basic Pension Number notification, etc.)

Notification of Pregnancy

Required Information for Pregnancy Notification Form

  • Name of the pregnant woman, personal number, date of birth, address, phone number, occupation
  • Current gestational week, expected delivery date
  • Presence or absence of health checkups, name and location of the medical institution where pregnancy diagnosis was received

For those who wish to have an interview at the same time as submitting a pregnancy notification

At the Parent and Child Comprehensive Support Center, you can have an interview with a Public Health Nurse at the same time as your notification (also available on Saturdays).

If you wish, please make a reservation in advance from the following.

When you have an interview, we provide items such as the "Childcare Package" and "Pregnant Woman Gift".
