The Notification Card is a paper card that informs you of your 12-digit individual number (My Number).

The Notification Card is used as a document to prove your My Number when you are asked to present it for social security, tax procedures, and other such matters.

  • Note: The Notification Card can only be used as a document to prove your My Number if the information on the card (such as address and name) matches the Certificate of Residence.
  • Note: This card includes your name, address, date of birth, and gender, but since it does not have a photo, it cannot be used alone as an identification document.

The design of the card is as follows.

The notification card system has been abolished (changes to the information on the notification card and reissuance are no longer possible)

Due to the amendment of the law, the notification card system has been abolished, and as of May 25, 2020, changes to the information on the notification card (such as address and name) and reissuance are no longer possible.

Even after the notification card system has been abolished, if the information on the notification card (such as address and name) matches the Certificate of Residence, it can continue to be used as a document to prove the My Number.

In addition, you can still apply for a My Number Card even after the notification card is abolished.
