Change of My Number Card Design for Those Moving Abroad (Name Change)
If the name on the My Number Card of a person moving abroad changes, a procedure for changing the card face is required.
Procedures can be completed at the municipality of your registered domicile.
Application Location (Registered Domicile in Inagi City)
Inagi City Hall Citizen Affairs Division, Hirao Branch Office, Wakabadai Branch Office (Procedures can be completed on the spot)
Note: If you apply at an overseas diplomatic mission, this will be information regarding the reissuance application for the My Number Card.
Required Documents (Permanent Residence in Inagi City)
Personal Application
My Number Card (PIN verification is required at the counter)
Application by Legal Representative (for individuals under 15 years old or adults under guardianship) / Application by Spouse
- The individual's My Number Card (PIN verification is required at the counter)
- One official photo ID issued by a government office of the legal representative or spouse1 item (such as a passport or My Number Card)
- Certificate of Registered Matters (for adults under guardianship)
Points to Note
- Those who cannot enter the 4-digit PIN need to reset their PIN. For details, please refer to "My Number Card PIN Change and Reset for Those Moving Abroad."
- The signature electronic certificate embedded in the My Number Card becomes invalid if the name is changed. When issuing a new one, the individual must visit the office to complete the procedure (verification of two types of PINs, the 4-digit PIN and the signature electronic certificate PIN, is required).
