Application for My Number Card for Those Moving Abroad

Japanese nationals who have moved abroad on or after October 5, 2015, are eligible to apply for the My Number Card for Those Moving Abroad.
The application method is as follows. Please see the following link for information on receiving the card.

Application Location

Please apply by visiting one of the following locations or by mail.

  • Overseas diplomatic missions
  • Municipality of temporary residence
  • Municipality of registered domicile

Note: If you are reissuing the My Number Card due to damage or deterioration, you cannot apply by mail.

Required Documents [New Application / Renewal Application]

  • Application Form for Individual Number Card Issuance and Electronic Certificate Issuance/Renewal (Attach a passport-sized photo)
  • Individual Number Card / Electronic Certificate PIN Setting Request Form
  • Note: When an optional representative applies, the person themselves must fill out the PIN setting request form, then place it in an envelope and seal it with glue or similar so that the representative cannot see the PIN, and bring it in that sealed state.
  • Note: Please check the photo specifications in the "Photo Checkpoints (My Number Card Official Site)".
  • Note: Please download the Application Form from the "[Overseas] Application Form Download (My Number Card Official Site)".

  • Individual Number Card Reissue Application Form and Electronic Certificate Issuance/Update Application Form (Attach a passport-sized photo)
  • [For those who have lost it only] Documents proving the loss of the My Number Card (such as a lost property certificate)
  • [For damaged cards only] Damaged or defective My Number Card
  • Note: If the document certifying the fact of loss is issued in a foreign country, please attach a Japanese translation.

Fee

Free
However, a fee of 1,000 yen (800 yen if the electronic certificate is not installed) will be charged in the following cases.

  • Reissuance due to loss, damage, contamination, or destruction of the My Number Card
  • Reissuance after voluntary return of the My Number Card, etc.

For those whose application location is an overseas diplomatic mission or temporary residence, and the receipt location is an overseas diplomatic mission (if Inagi City is the registered domicile)

As soon as the documents such as the application form are confirmed in Inagi City, we will inform you about the payment method via email.
Those whose registered domicile is Inagi City, please make the payment by either method 1 or 2.
Note: Since payment methods vary depending on the municipality, those whose registered domicile is in another city, ward, town, or village should check with their registered domicile.

  1. Direct payment at City Hall (payment at Inagi City Hall by relatives or friends residing in Japan)
    Please inform us of the name and date of visit of those who will come to the office in advance.
  2. Sending cash by mail (registered cash mail, fixed amount postal money order)
    • If you send cash, please send it in Japanese yen (please send the exact amount without requiring change).
    • If you are sending a fixed amount postal money order (Japanese), please send one that was issued within the last 6 months.

Note: Do not write your name or any other information on the fixed amount postal money order. Also, payment by stamps or similar means is not accepted.

Period from Application to Receipt

The period from application to issuance of the My Number Card is approximately two months (as a guideline). If you have not received the issuance notification email more than three months after applying, please contact your registered domicile.
