Special Disability Allowance
Eligible Individuals
Persons aged 20 or older with a significantly severe mental or physical disability who require constant special care in daily life
Amount Granted
Monthly amount: 29,590 yen (Revised from April 2025)
Payments for three months up to the previous month are made around the 10th of May, August, November, and February
(There are some exceptions, such as for those newly certified or those who have lost their eligibility)
Degree of Disability Eligible
Persons who fall under any one of the following 1 to 4.
- Persons who have two or more disabilities specified in items 1 to 7 below.
- Persons who have one disability specified in items 1 to 7 below, and in addition, have two other disabilities equivalent to Grade 2 of the Disability Basic Pension, totaling three disabilities.
- A person who has one disability specified in items 3 to 5 below, and because it is particularly severe, the evaluation of their ability to perform daily living activities is extremely severe
- A person who has one disability specified in items 6 to 7 below, and whose condition requires absolute rest or, in the case of a mental disability, the evaluation of their daily living ability is extremely severe

- The following visual impairments
- Visual acuity of 0.03 or less in both eyes
- Visual acuity of 0.04 in one eye and light perception or less in the other eye
- As a result of measurement with a Goldmann perimeter, the sum of the peripheral visual field angles with the I/4 target in both eyes is 80 degrees or less, and the central visual field angle with the I/2 target in both eyes is 28 degrees or less
- As a result of measurement by an automated perimeter, those with a binocular open visual recognition score of 70 points or less and a binocular central visual field recognition score of 20 points or less
- Those with a hearing level of 100 decibels or more in both ears
- Those who have severe functional disabilities in both upper limbs, or who lack all fingers on both upper limbs, or who have severe functional disabilities in all fingers of both upper limbs
- Those who have a severe disability in the function of both lower limbs or who are missing both lower limbs at or above the ankle joint
- Those who have a disability in trunk function to the extent that they cannot sit or stand up
- In addition to those listed in the preceding items, a condition recognized as having a physical functional disability or a medical condition requiring long-term rest that is equal to or greater than the preceding items, to the extent that it makes it impossible to manage daily life
- A mental disability recognized to be equal to or greater than the preceding items
Payment Restrictions
Those who fall under any of the following categories cannot apply or receive benefits. For more details, please contact the Disability Welfare Division.
- Individuals whose income exceeds the income limit (Note)
- Individuals residing in a facility
- Individuals hospitalized in a hospital or clinic for more than 3 months
Note: Whether the income exceeds the income limit is determined based on the amount after deductions from the income of the person themselves or their spouse/dependent obligor. For details, please refer to the income limit table and deduction table below.

If the following conditions apply, you can receive an increase in the limit amount.
- Income of the beneficiary
- When there is an elderly spouse eligible for deduction or an elderly dependent among the dependents, 100,000 yen per person
- When there is a specified dependent or a dependent eligible for deduction (limited to those aged 16 or older but under 19) among the dependents, 250,000 yen per person
- Income of Spouse and Dependents (in cases where the number of dependents is two or more)
When there is an elderly dependent among the dependents, 60,000 yen per person (if there are no other dependents besides the elderly dependent, 60,000 yen per elderly dependent excluding one person)

Deductions for the above items are made from the income of the individual, spouse, or supporting obligor, and income assessment is conducted.

- Application for Special Disability Allowance Certification
- Special Disability Allowance Income Status Report
- Special Disability Allowance Certification Medical Certificate (Designated Form)
- Application for Direct Debit of Special Disability Allowance (Please specify the account in the name of the applicant)
- Note: Those who are receiving the Severe Physical and Mental Disability Allowance do not need to submit a medical certificate.
- Note: There are fields to enter the personal number (My Number) in 1 and 2.

- Declaration Regarding Deductible Dependents Aged 16 to Under 19
- Copy of the Pension Certificate of the Beneficiary (if receiving Disability or Survivor's Pension)
- (Non) Taxation Certificate
