Notice of [1-Year-Old Gift]

Around the age of 1, children's range of activities expands, and their self-assertion becomes stronger, leading to many parenting concerns and inquiries. The city will send a survey about parenting to families raising children who have celebrated their first birthday, and to those who respond, we will distribute a 60,000 yen gift certificate for children's products as a first birthday present.
Please note that we may contact you regarding the contents of your survey responses.

Eligible Individuals

Households with children who meet all of the following requirements

1. Children born on or after April 2, 2023
2. Children who turn 1 year old and are registered as residents in Inagi City at that time
3. Children who have their resident registration in Inagi City at the time the survey arrives at the city before their second birthday.

Implementation Period

From April 1, 2025 to March 31, 2030

Regarding the survey response and the presentation of the 1-year-old gift

(1) In the case of the first child or a child born from multiple births

Please fill out the "1-Year-Old Dental Health Checkup Questionnaire" and submit it at the reception for the 1-Year-Old Dental Health Checkup. In exchange for the questionnaire, you will receive a gift for your 1-year-old.
Note: If you do not receive the Dental Health Checkup for 1-year-olds, please return the "1-Year-Old Dental Health Checkup Questionnaire" using the enclosed return envelope. After receiving the questionnaire, it will be mailed to your registered address within approximately 2 to 3 months.

(2) For the second and subsequent children

Please fill out the "1-Year-Old Dental Health Checkup Questionnaire" and return it using the enclosed reply envelope. After receipt, it will be mailed to your registered address within approximately 2 to 3 months.
Note: The target for the Dental Health Checkup for 1-year-old children is the first child and children born from multiple births.
