Electronic Application for Fire Prevention Notifications
You can submit notifications related to fire prevention using the LoGo form
You can submit notifications anytime, 24 hours a day, 365 days a year using the LoGo form from your computer or smartphone
Note: Depending on the amount of attached document data during the application, it may not be possible to attach files due to system limitations. In such cases, please prepare and submit the various notification forms to the Fire Department Prevention Division.
Note: Files with extensions exe, bat, and sh cannot be attached.
Note: The maximum file size for a single attachment is 10MB.
Note: The maximum file size for attachments in a single response is approximately 100MB.

Handling of Copies
If there are no deficiencies in the application after electronic submission, we will return the notification form as an electronic document, clearly stating the reception date and reception number. Please check the procedure email from the Fire Department Prevention Division. However, only the notification form will be returned, so if there are any attached files, please keep the ones you attached at the time of application.
