These are the forms for the invoice and payment account transfer request to be submitted to the city.
Please download and use as needed.

Note: From April 1, 2024, seals can be omitted on invoices and payment account transfer request forms.
For details on the omission of seals, please see the following link.

Invoice

Notice: The Excel format invoice does not contain any formulas for calculating amounts.
Please enter the tax rates, calculation formulas, etc. based on the contract as needed and make use of them.

However, invoices can be submitted in any format as long as the required information is included.

<Required Items on the Invoice>

  1. Creditor (Address, Name; in case of a corporation, the corporation name and the representative's name)
  2. Debtor (Addressed to the Mayor of Inagi City)
  3. Invoice Amount
  4. Details of the Invoice Contents
  5. Invoice Date
  6. Payment Destination
  7. Seal (Optional)
  • Note: The Invoice System started on October 1, 2023; however, as a general rule, the city does not require business operators to issue qualified invoices (invoices). Nevertheless, for Sewerage Operations Accounting and Hospital Business Accounting, it is necessary to receive qualified invoices from consumption tax taxable business operators who are qualified invoice issuers.

Payment Account Transfer Request Form (Creditor Registration)

  • When receiving payments from Inagi City, you can register the bank account for receiving funds.
    A creditor code will be issued upon registering the transfer destination.
    By entering the creditor code on the invoice, payments will be transferred to the registered account, saving you the trouble of specifying the transfer account each time you invoice.
  • If you wish to register as a new user, please fill in the required information on the Payment Account Transfer Request Form and submit it to the relevant departments.
    If you are already registered as a creditor with the Mayor of Inagi City and there are changes to your registration details, please submit the Payment Account Transfer Request Form in the same way as for new registration.
