Mental Disability Welfare Handbook
This is a certificate necessary for individuals with mental disabilities to receive various forms of support, issued by the governor of the prefecture (hereinafter referred to as "certificate" on this page).
This is a certificate necessary for individuals recognized as having a mental disorder as defined by the laws concerning mental health and welfare for persons with mental disabilities, to receive various support systems for daily and social life. It is issued in grades from 1 to 3 based on the state of mental illness (mental disorder) or the degree of disability. Additionally, those who already possess a certificate and are recognized as having a change in the degree of their disability can apply for a grade change.
Application Method
Please bring the following items and apply at the Disability Welfare Division counter or by mail.
- Application Form
- Medical certificate (limited to those that have passed more than 6 months from the initial diagnosis date related to mental disabilities (as of the time of certificate creation). The form can also be downloaded from the Tokyo Metropolitan Central Comprehensive Mental Health Welfare Center website.)
Note: If you are receiving a disability pension based on a mental disorder, a medical certificate is not required if you submit a consent form along with one of the following documents.- Copy of Disability Pension Certificate
- Copy of Disability Pension Decision Notification
- Copy of Disability Pension Payment Notification
- Photo (4cm tall x 3cm wide. No hat, upper body, taken within one year from the application date) 1 piece
- My Number Card Related Documents
- Copy of your notebook (in case of renewal)
- Return envelope (only for mail applications. Please affix a 110 yen stamp)
About Application Forms
If you wish to have the necessary application documents sent to you, please apply using the form below. It takes about two weeks from application to delivery, so if you are in a hurry, please visit the Disability Welfare Division counter directly.
Notes on Updates
The validity period is 2 years.
You can apply for renewal starting three months before the expiration of the validity period.
We provide updates every month through the Inagi City email distribution service.
Other Topics
- Based on the application, an examination will be conducted in Tokyo, and if the grade is determined, the handbook will be issued. Depending on the examination results, the handbook may not be issued, or a different grade may be determined compared to before the renewal.
- It takes about 3 months until the handbook is issued.
- When applying for both the disability certificate and the self-support medical care (mental outpatient) at the same time, it may be possible to apply using only the diagnosis certificate for the disability certificate.
