Supportive Medical Care (Mental Outpatient)
This system provides subsidies for Medical Expenses incurred from continuous outpatient treatment for mental disorders. The standard personal burden is 10%. However, depending on the individual's income, the insured household's income, the disease, etc., a monthly personal burden limit may be set, or the entire personal burden amount may be exempted.
Home visits, day care, home nursing, epilepsy treatment, and medication costs related to outpatient psychiatric medical care are also covered. Medical expenses related to hospitalization are not covered.
Application Method
Please bring the following items and apply at the Disability Welfare Division counter or by mail.
- Application Form
- Medical certificate (can also be downloaded from the Tokyo Metropolitan Central Comprehensive Mental Health Welfare Center website.)
- Note: Must be created by a physician within 3 months.
- Note: Since it is submitted once every two years, it may not be necessary to update it.
- Any of the following insurance information verification documents (not required for those under the Inagi City National Health Insurance or Late-Stage Elderly Medical Care system.)
- Copy of Health Insurance Card
- Copy of the "Eligibility Confirmation Document" issued by the health insurer
- Copy of the "Qualification Information Screen" that can be checked from the My Portal
Note 1: If the patient is enrolled in a National Health Insurance Union (such as the National Civil Engineering and Construction National Health Insurance Union or the Tokyo Barber National Health Insurance Union), insurance information verification documents for all household members enrolled in the same insurance as the patient are required.
Note 2: If none of the above can be prepared, we will verify by making an inquiry through My Number linkage, which may take some time.
- Household Income Statement and Consent Form
- Certificate of Eligibility for Medical Support for Independence (Mental Health Outpatient) (for Renewal)
- Taxation (Non-Taxation) Certificate (Required for those moving in, if necessary. Please contact us in advance.)
- Documents that confirm the name and location of medical institutions and pharmacies (such as medical examination tickets and medicine bags)
- Documents related to My Number
- Identification document (If an agent is applying, the agent's identification document is also required)
- Return envelope (only for mail applications. Please affix a 110 yen stamp)
Note: After the issuance of the Certificate of Eligibility, it will basically be sent to the recipient's home. However, if a third party must come to the Disability Welfare Division counter to receive it due to unavoidable circumstances, please bring the "Power of Attorney (Disability Welfare Division Form)" below and the agent's identification documents. Note that a third party refers to a person who does not fall under any of the following categories (the recipient themselves, the parent or guardian if the recipient is under 18 years old, or a legal representative).

Notes on Updates
The upper left corner of the beneficiary certificate indicates whether a medical certificate is required or not.
If it states that it is required, please request the medical institution to prepare the medical certificate.

The validity period is generally one year.
You can apply for renewal starting three months before the expiration of the validity period.
We provide updates every month through the Inagi City email distribution service.

If this renewal is a "no medical certificate required" case and the application is within the validity period, you can apply for renewal online.

If you wish to apply online, please fill in the required information in the online application data below and upload it to the application form in (2).
- Please be sure to operate in a PC environment (if opened on a mobile phone, some Excel functions may not be available, and it may not function properly).
- If it is difficult to prepare an environment for creating and uploading Excel files, please apply at the service counter.
- Please do not enter your Individual Number (My Number).

If you wish to apply online, please proceed to the application form below and upload the (1) "Self-Support Medical Care (Outpatient Psychiatric Care) Online Application Data (No Medical Certificate Required)".
Note: If you have an upcoming medical appointment soon and need to receive the "Applicant's Copy" urgently, please apply directly at the Disability Welfare Division counter.
In the case of online applications, the "Applicant's Copy" will be sent by mail approximately 10 days after your application is received.
If you need to show the "Applicant's Copy" to a medical institution at your next visit and the scheduled date of that visit is soon, or if you need the "Applicant's Copy" urgently, please apply directly at the Disability Welfare Division counter.
Note: In the case of an in-person application, the applicant's copy will basically be handed to you on the spot at the time of application.
Other Topics
- Based on the application, an examination will be conducted in Tokyo, and if approved, a certificate for self-supporting medical care (mental health outpatient) will be issued.
- It takes about 3 to 4 months until the recipient certificate is issued.
- Can only be used at designated medical institutions listed on the recipient certificate.
- If there are no changes in the medical condition and treatment policy, the submission of the Self-Support Medical Certificate will be required once every two years. However, if the application is made after the validity period has expired, it will be considered a reapplication, and the submission of the certificate and other documents will be necessary.
- When applying for both Independence Support Medical Care (Mental Outpatient) and the Mental Disability Welfare Handbook at the same time, it may be possible to apply using only the diagnosis certificate for the handbook.
- Updates, changes to medical institutions, changes to health insurance, changes of address, changes of name, and reissuance due to loss or damage require procedures.
- If you have a Mental Disability Welfare Handbook issued based on a diagnosis certificate, you may be able to submit a copy of the handbook instead of the diagnosis certificate when applying. For more details, please contact us.
