FY2026 Health Checkup

The implementation will continue until Saturday, October 31, 2026.

We conduct Health Checkups under the Late-Stage Elderly Medical Care system.
Examination tickets will be sent sequentially to eligible individuals.

How to Receive Medical Examination

1. Please make a reservation directly with the medical institution conducting the health checkup.
   Note: Please say "I would like to make a reservation for the Health Checkup for Late-Stage Elderly Medical Care."
2. Please fill out the back side (questionnaire) of the Late-Stage Elderly Medical Care Health Checkup examination ticket in advance.

Items to Bring When Visiting

Please bring the following two items.

1. Late-Stage Elderly Medical Care Health Checkup Examination Ticket
2. Something that can verify insurance eligibility (either the My Number Card linked with the Health Insurance Card or a Certificate of Eligibility)
