Notification of Changes for Pet Dogs

Please complete the procedures at the contact point below for the location of your pet dog, the owner's address and name, any changes to the owner, or if the pet dog has passed away.

• Note: The available procedure hours are from 8:30 AM to 5:00 PM on open days.

Please submit the "Change Notification for Registered Dogs."

Belongings

Dog license issued at the time of registration

• When moving into Inagi City, please bring the license issued by your previous municipality. If it has been lost, please prepare a reissue fee of 1,600 yen.
• If you are moving out of Inagi City, no procedures are required in Inagi City. Please bring your Inagi City certificate and apply to the municipality you are moving to.
• Applications by mail will be accepted only from those who have a certificate issued by the previous municipality.
  Please print and fill out the form below, attach your license, and send it to the Inagi City Health Center. After confirmation by the person in charge, we will send you the Inagi City license.

Procedure for when a registered dog has passed away

Please submit a "Dog Death Notification" when a registered dog has passed away.

Applications by phone are also accepted (Inagi City Health Center only).
Applications can also be submitted via the LOGO form. Please apply using the link below.
