Tokyo Metropolitan Severe Physical and Mental Disability Allowance
Eligible Individuals
(1) Individuals with severe intellectual disabilities who require constant supervision by a caregiver due to significant mental conditions and need special consideration
(2) Individuals with overlapping severe intellectual and severe physical disabilities
(3) Persons with severe physical disabilities who have lost function in both upper and lower limbs and have disabilities to the extent that sitting is difficult or worse
Note: Simply being classified as severe (Physical Disability Certificate grades 1 or 2, Ai no Techou grades 1 or 2) does not meet the eligibility requirements for payment.
Note: It is necessary to undergo a direct assessment at the Tokyo Welfare Center for the Physically and Mentally Disabled or the Tama Branch Office. If it is difficult to visit due to the disability situation, an assessment can also be conducted at home.
Amount Granted
Monthly amount: 60,000 yen
Payment Restrictions
Those who fall under any of the following categories are not eligible to apply or receive benefits. For more details, please contact the Disability Welfare Division.
(1) When income exceeds the income limit standard amount (for those 20 years and older, the individual's income; for those under 20, the income of the person obligated to support)
(2) When residing in a facility
(3) When hospitalized continuously for more than 3 months in a hospital, etc.
(4) Those who are 65 years or older and applying for the first time
Note: Whether the income exceeds the income limit is determined based on the amount after deductions from the income of the person themselves or their obligated supporter. For details, please refer to the income limit table and deduction table below.

If the following conditions apply, you can receive an increase in the limit amount.
- If there is an elderly spouse eligible for deduction or an elderly dependent relative among the dependents, 100,000 yen per person
- If there is a specified dependent (19 years or older but under 23) or a dependent eligible for deduction (16 years or older but under 19) among the dependents, 250,000 yen per person

Note: The (Non) Taxation Certificate can be omitted if you enter the Individual Number (My Number) in item 1.
4. Power of Attorney (Disability Welfare Division Form)
Note: If a third party is applying on your behalf, please bring the "Power of Attorney (Disability Welfare Division Form)" and identification documents of the representative. A third party refers to a person who does not fall under any of the following categories: the applicant themselves or the person legally responsible for the applicant if the applicant is under 20 years old.
