About the Medical Expenses Subsidy System for Children in Inagi City

Eligibility

Infants and Young Children Medical Expenses Assistance Program (commonly known as Maru-nyu)

Infants and Young Children residing in Inagi City who are enrolled in health insurance (until the first March 31 after reaching 6 years old)

Medical Expenses Assistance System for Compulsory Education Children (commonly known as: Maruko)

Children of compulsory education (from 1st grade of elementary school to 3rd grade of junior high school, until March 31 of the year they turn 15) residing in Inagi City and enrolled in health insurance

High School Student Medical Expenses Assistance Program (commonly known as: Maru Ao)

High school students residing in Inagi City who are enrolled in health insurance (from the first April 1 after reaching the age of 15 until the first March 31 after reaching the age of 18)

Notes on Each System

  • There are no income restrictions.
  • Households receiving Public Assistance, individuals residing in facilities designated by regulations, and those entrusted to foster parents will have their respective Medical Expenses subsidy programs prioritized. Additionally, individuals receiving the single-parent Medical Expenses subsidy program and the medical expense subsidy program for persons with physical and mental disabilities without any out-of-pocket costs (those from non-taxable households) will have their respective Medical Expenses subsidy programs prioritized.
  • The applicant (Parent or Guardian) will be one of the parents or guardians of the infants and young children, primarily the one who supports the household financially (the one with the higher income).

Scope of Subsidy

Infants and Young Children Medical Expenses Assistance System

The city will fully subsidize the portion of Medical Expenses that you must bear yourself, which is covered by health insurance.

Medical Expense Assistance System for Compulsory Education Students and High School Students

We provide subsidies for 30% of Medical Expenses covered by health insurance that are paid out-of-pocket, as described below.
Outpatient visits: Subsidies cover the portion exceeding 200 yen per visit (maximum out-of-pocket expense 200 yen)
Hospitalization and Dispensing: Full Subsidy (No Out-of-Pocket Expense)

Notice

  • Expenses not covered by health insurance, such as health checkups, vaccinations, orthodontics, meal costs during hospitalization, additional bed fees, and initial consultation fees for non-referred patients at Inagi City Hospital, will not be subsidized.
  • If there are high medical expenses or additional benefits provided by the health insurer (such as health insurance associations), the amount after deducting high medical expenses and additional benefits from the out-of-pocket expenses for insured medical care will be subsidized.

Subsidy Start Date

The start date of the subsidy eligibility will be from the first day of the month in which the application was submitted.
Note: For applications due to birth or moving in, if the application is submitted by the same day of the following month after the birth date or moving-in date, the start date of the subsidy eligibility will be the birth date or moving-in date, even if the month has changed.

A child was born / Moved in from another municipality

New Applications (Birth, Transfer, etc.)

Please bring or send the following documents.

  1. Application Form for Medical Certificate (You can download it from the link below or it is also available at the counter)
  2. Qualification confirmation document, etc. of the health insurance union to which the child in question is enrolled (or is scheduled to be enrolled)
  3. Documents showing the branch name and account number of a financial institution (in the name of the applicant or spouse, for a regular savings account)
  4. Documents that confirm the health insurance of the eligible child (such as qualification confirmation documents) are available.
  5. My Number Card Verification Document
    When applying, it is necessary to include the My Number on the Application Form. (For the applicant and spouse)

After the application form arrives, it will be sent to your home in about two months.

Continuation Procedures

Fiscal Year Transition

The fiscal year changes every October.
An annual review will be conducted continuously. After the review, those who need document verification will receive a letter from August each year, so please follow the instructions and complete the procedures.
For those whose qualifications continue, a new medical certificate starting from October 1 will be mailed.

Switching of the system

Every year in April, new first graders who have a Maru milk medical certificate will receive a Maru child medical certificate, and those who have a Maru child medical certificate and are equivalent to new first-year high school students will receive a Maru blue medical certificate by mail in late March. No procedures are required for the transition of the system.

How to Use the Medical Certificate

When receiving medical treatment, subsidies are provided by submitting the eligible child's medical certificate and qualification confirmation documents together at the medical institution's reception.

When receiving medical treatment at a medical institution outside the prefecture

We will reimburse the Medical Expenses you paid to medical institutions (your out-of-pocket share for insured medical treatment).

Notice

  • The application deadline is within 5 years from the day after the payment of Medical Expenses and is limited to those within the validity period of the medical certificate.
  • If there are other special medical expenses, additional documents may be required, so please contact the Child Care Support Division.
  • It takes about 2 months from application to payment (bank transfer).

In the case of receiving benefits based on the Japan Sports Promotion Center Law

If your child becomes ill or injured at school, Kindergarten, Nursery School, etc., benefits based on the Japan Sports Promotion Center Act may be provided. If instructed by the school or similar institution, please visit the hospital without using the medical certificate. (If no benefits are provided, please apply at the city office counter.)

Injured due to the actions of others (third-party actions)

If you are injured due to the actions of others and have used your medical certificate along with the qualification confirmation document, please contact the Child Care Support Division at City Hall.
We will send documents to the perpetrator to claim costs from Inagi City.

When You Lose Your Medical Certificate

Lost or damaged medical certificate

Please submit the Medical Certificate Reissuance Application Form. (You can download it from the link below or it is also available at the service counter.)
A new medical certificate will be mailed to you at a later date.
(If lost, a medical certificate is not required)

When You Move

Moved within Inagi City

Please submit the Medical Certificate Application Change Notification. (You can download it from the link below or it is also available at the service counter.)
A new medical certificate will be mailed to you at a later date.
When you receive a new medical certificate, please return the old one.

Please return your medical certificate after moving out.
You will need to submit a new application to the municipality you are moving to.

Note: The Medical Expenses assistance system is a program conducted independently by each municipality. Please check the system and required Application Form documents at your new location.

When the health insurance you are enrolled in changes

Notification of insurance changes is required

Please bring the following documents and apply at the Child Care Support Division, 2nd floor of City Hall, or at the Hirao or Wakabadai Branch Office.

  • Documents that confirm the health insurance of the eligible child (such as qualification confirmation documents) are available.

Points to Note When Submitting by Mail
The acceptance date by mail will be the date the application form or notification arrives at City Hall.
If the arrival date at City Hall is delayed due to postal accidents, the effective date of eligibility for benefits may be delayed (the month you start receiving allowances or the start of the validity period of the medical certificate may be delayed). To ensure that the Application Form or notification reliably reaches City Hall, please send it using methods such as specified recorded mail or simplified registered mail.
If there are any deficiencies in your application or notification, we will contact you later by phone or letter. Please be sure to provide your address, name, and phone number.
