Online Application for Certificate Issuance

Those who have either a My Number Card, a Special Residence Card, or a Special Permanent Resident Certificate (with a valid electronic signature certificate installed) can apply online for certificate issuance.

After applying online, certificates and other documents will be mailed from the city.

Eligible Applicants

Persons who possess any of the following

1. My Number Card
2. Special Residence Card (with valid My Number Card function part)
3. Special Permanent Resident Certificate (with valid My Number Card function part)

• Only those with a valid signature electronic certificate are eligible. When applying, it is necessary to enter the PIN for the signature electronic certificate (an alphanumeric code of 6 to 16 characters).
• It is necessary to enter the secret code for ticket information input assistance (a 4-digit number).
• If you have not updated the card information on your My Number Card, etc., during past moves or other changes, you cannot use the online application.
