Note: For online applications, you need one of the following: My Number Card, Special Residence Card, or Special Permanent Resident Certificate (with a valid signature electronic certificate installed). If you do not have a My Number Card, etc., please apply at the counter or by mail.

Note: For online applications, you need to enter the ticket information input assistance PIN (4-digit number) and the signature electronic certificate PIN (6 to 16 alphanumeric characters).

• If you have not updated the card information on your My Number Card, etc., during past moves or other changes, you cannot use the online application.

1. Install the dedicated app (Electronic Authentication My Number Sign) on your smartphone in advance.
2. Access the online application form and register your email address.
3. A login URL will be sent to the registered email address.
4. Log in to the application form again from the URL and fill in the required information.
5. After confirming the input details, you will perform an electronic signature. Read the My Number Card using a dedicated app on your smartphone and press the application button.
6. After confirming the application details, payment will be made on the payment screen.
7. After reviewing the application details and confirming the payment information at the Citizen Affairs Division, the certificate will be mailed.

You can download the dedicated app (Electronic Certification MySign) from the link below.

This is a dedicated app for reading signature electronic certificates. It is used during the online application process for the certificate.

Note: Only smartphones compatible with the My Number Card, etc., are supported.

Note: To use the online application, it is necessary to install the dedicated app (Electronic Authentication My Number Sign).

Note: When searching and installing from the App Store or Google Play, searching for "My Sign" will display the download site.

Notes

• Applicants who possess a My Number Card, etc. (with a valid signature electronic certificate installed) can request their own certificates.
• Identity verification using the My Number Card is required. You need to enter the PIN for the card information input assistance (4-digit number) and the PIN for the signature electronic certificate (6 to 16 alphanumeric characters).
• The certificate will be issued in writing and mailed, so you will also be responsible for the postage.
• Those living abroad cannot use the online application.

• The person listed on the Certificate of Residence and members of the same household can apply. (Applications from third parties are not allowed)
• The removed Certificate of Residence (Johyo) can be applied for by the individual only.
• You cannot apply for the removal of the deceased from the registry.
• The Certificate of Residence code cannot be entered.

• The person listed on the Certificate of Residence and members of the same household can apply.
• The Certificate of Residence details certificate will be in the city's format.

Send

• In addition to the certificate issuance fee, you will also be responsible for a postal fee of 110 yen.
• If the postage is insufficient due to excess weight, it will be sent as "insufficient amount paid by the recipient."
• The certificate will be sent approximately 3 business days after the review is completed by the city staff.
• The certificate will only be sent to the address listed on the My Number Card or equivalent.
• The certificate will be mailed to your home without forwarding. If you have forwarding set up, you will need to cancel the forwarding procedure.

Other Topics

• Those eligible for support measures such as DV cannot apply online. (Please apply at the counter)
• Online applications do not qualify for fee exemptions (due to reasons such as receiving Public Assistance). Additionally, refunds after application cannot be accommodated.
