From December 2, 2024, the system has been based on using the My Number Card as a Health Insurance Card, called the My Number Health Insurance Card.
For those who have a My Number Health Insurance Card, a Qualification Information Notice is issued, and for those who do not have one, a Qualification Confirmation Document is issued. Therefore, please present either the My Number Health Insurance Card or the Qualification Confirmation Document at medical institutions when receiving medical care.
Note: People aged 75 and over are covered by the Late-Stage Elderly Medical Care system (not National Health Insurance).

Expiration Date of the My Number Health Insurance Card
With the My Number Card Health Insurance Card, qualification verification at medical institutions, etc., is conducted using the user authentication electronic certificate of the My Number Card (hereinafter referred to as the "electronic certificate"). However, since the validity period of the electronic certificate is until the fifth birthday after its issuance, it is necessary to complete the renewal procedure before the electronic certificate expires.

The validity period of the qualification confirmation document is as stated on the card.
For those whose period of stay expires by July 31, 2027 = Until the period of stay expires
For those other than the above = Until July 31, 2027
You may dispose of expired qualification confirmation documents yourself, taking care to protect personal information.
For those aged 70 to 74
For those aged 70 to 74, the self-payment ratio will be either 30% or 20% depending on income. Until December 2024, a certificate for older adults indicating the self-payment ratio was issued, but with the My Number Health Insurance Card, the self-payment ratio can be confirmed online, and the qualification confirmation document also states the self-payment ratio. Therefore, please present either the My Number Health Insurance Card or the qualification confirmation document when visiting medical institutions.
Validity Period of Qualification Confirmation Document and Qualification Information Notice (For Those Aged 70 to 74)
The validity period of the qualification confirmation document and qualification information notice is until July 31, 2027. (For those who will turn 75 years old by July 31, 2027, it is until the day before their birthday.)
If there is a change in the burden ratio before the expiration date, a new qualification confirmation document and qualification information notice will be sent again.
You may dispose of expired qualification confirmation documents and qualification information notices yourself, taking care to protect your personal information.

- Even for those who fall under the 30% burden category, if the city can confirm that the total income of National Health Insurance members aged 70 and over in 2024 is less than 3.83 million yen for one person, or less than 5.2 million yen for two or more people, the burden will be 20%.
- For National Health Insurance members aged 70 and over, if the total "amount of income after basic deduction" is 2.1 million yen or less, the burden will be 20%.
