Request for Mailing of Tax Certificate and Non-Tax Certificate

When requesting a Taxable (Non-Taxable) Certificate by mail, please enclose the following four items and send them to the Taxation Division.

1 Mail Request Application Form

Please obtain the application form from the following application form download page. You may also write it on letter paper following the example of the obtained mail request application form.

2 Fixed Amount Postal Money Orders (for the number of requests)

Please purchase it at the post office as it is required as a fee.

1. For mail applications, the certificate costs 400 yen per copy. Please be sure to enclose the exact amount corresponding to the number of copies requested, with no change required.
2. The validity period is 6 months from the date of issue. For administrative reasons, we kindly ask you to send a fixed amount postal money order that has 10 days or more remaining validity.
3. Please send the fixed amount postal money order unfilled on both sides.

3 Return envelope (with return postage attached and recipient address filled in)

4 Copy of identification document (if an agent, the agent's identification document)

5 Power of Attorney (when an agent applies)

The signature of the certifying person is required.
Note: If the application form is received when you are not a resident of Inagi City, a power of attorney is required even for cohabiting relatives.
