Documents to Submit After Starting Use

When you start using a Childcare Center, please submit the following documents to the Child Care Support Division, Childcare and Kindergarten Admission Certification Section.

20th of the month following the month when use begins

If you are unable to return to work by the end of the month when use begins, your job offer will be canceled or you will be discharged.

Also, if the required documents are not submitted by the deadline without a valid reason, or if the submitted information differs from what was provided at the time of application, the termination will be effective at the end of the submission deadline month.

Employment Offer
  1. Notification of Change in Family Situation
  2. Employment Certificate (dated on or after the employment start date)
20th of the month following the month when use begins

The last day of the second month after the month service starts If you cannot start working within three months including the month you begin using the service, you will be discharged.

Procedures Required Due to Changes in Family Situation

In the following cases, please submit the required documents to the Child Care Support Division, Childcare and Kindergarten Admission Certification Section.

Start of Employment, Job Change, Change in Employment Details

  1. Notification of Change in Family Situation
  2. Employment Certificate
  3. Schedule Table (Self-Employed, etc. (Note: 1), When Working at Two or More Locations)
  4. Self-Employment Performance Documents (Note: 2) (Not required to submit if a corporation or if the business office is located outside the home)

 

Note: 1 Self-employed, employees of family-run businesses, contractors, freelancers, company executives, etc.

Note: 2 A copy of one of the following

  • Final Tax Return (Most Recent Year of Business Operation)
  • Documents showing income for the past 3 months (e.g., copy of bankbook)
  • Copy of Business License (Notification of Opening)

 

Note: Due to the work status survey, you may be asked to submit additional documents at a later date.

  • When changing the usage time category (short time / standard time)

Please submit 1 by the month before the month you want to change.

2 (and 3 and 4, if applicable) must be submitted promptly after the change.

  • If there is no need to change the usage time category

After making changes, please promptly submit 1 and 2 (and 3 and 4 if applicable).

 

Note: The minimum standard for the "employment" requirement is at least 3 days a week and at least 12 hours a week.

Employment Offer
  1. Notification of Change in Family Situation
  2. Employment Certificate (for proof after employment)
  • If you have secured employment

Please submit 1 by the month before the planned start month of employment, and submit 2 after starting employment.

  • Those who applied for and started using a Childcare Center under the condition of employment offer

Please submit 2 after starting work.

Please note that if you are unable to start working within the month you begin using the service, you will be required to leave.

Resignation

Notification of Change in Family Situation

Please submit by the month before the scheduled month of resignation.

If you are unable to start working within three months after resignation and there are no changes to other requirements, you will be required to leave.

Expected childbirth (Taking maternity leave before and after birth)
  1. Notification of Change in Family Situation
  2. Copy of the Maternal and Child Health Handbook (page with the names of the father and mother and the expected date of delivery) Note: In the Inagi City Maternal and Child Health Handbook, P1・4
  3. Certificate of Leave and Return to Work

Please submit items 1 and 2 by two months before the expected month of childbirth.

The certification period is the month of childbirth and the two months before and after it.

  • When Returning to Work After Childbirth

Please submit 1 by the month before the scheduled return month, and submit 3 (valid only on the proof date after returning to work) after returning to work.

  • When taking childcare leave after maternity leave

The procedure is the same as for "Taking Childcare Leave" below.

  • If the month of childbirth changes

Please submit 1.

Acquisition and Return to Work of Childcare Leave
  1. Notification of Change in Family Situation
  2. Certificate of Leave and Return to Work
  • When Taking Childcare Leave

[Mother's Childcare Leave Acquisition]

Please submit 1 and 2 by the second month after the month of childbirth.

[Paternity Leave Acquisition]

If the father takes childcare leave for one month or more, please contact us in advance.

The certification of the "childcare leave" requirement (shortened childcare hours) may be changed to coincide with the timing of the father's childcare leave acquisition.

In that case, procedures must be completed by the month before taking childcare leave.

 

Note: The usage time category for the older child during childcare leave is short-time childcare.

Note: If the child currently enrolled in the Childcare Center takes childcare leave, the child will be withdrawn from the center.

 

  • When Returning to Work

Please submit 1 by the month before your scheduled return to work, and submit 2 (valid only on the proof date after returning to work) after your return.

  • Those who have applied for and started using a Childcare Center in preparation for returning to work

Please submit 2 after returning to work.

If you are unable to return to work by the end of the month in which you start using the service while meeting the employment conditions at the time of application, your job offer will be canceled or you will be required to leave the facility.

Diseases and Disabilities
  1. Notification of Change in Family Situation
  2. Medical Certificate (Notes)
  3. Copy of Disability Certificate, etc.

Note 2 requires a description of the treatment period and the difficulty of home care.

Please promptly submit either 1 and 2 or 3.
Nursing and Caregiving
  1. Notification of Change in Family Situation
  2. Medical certificate of the person being cared for or nursed, copy of Disability Certificate, etc.
  3. Schedule

Please submit 1, 2, and 3 promptly.

 

Note: The minimum requirement for "nursing and caregiving" is at least 3 days a week and 12 hours a week.

School Enrollment
  1. Notification of Change in Family Situation
  2. Certificate of Enrollment (Issued by the School)
  3. Schedule

Please submit 1, 2, and 3 promptly.

 

Note: The minimum standard for the "school attendance" requirement is at least 3 days a week and at least 12 hours a week.

Change of residence within the city, name change, change in family composition (marriage, divorce, etc.)
  1. Notification of Change in Family Situation
  2. Certified copy of family register and other certificates (For details, see "Childcare Center Usage Guide" p.10)
  • In case of moving within the city or name change

After moving within the city or changing your name, please submit 1 promptly.

  • In cases of marriage, divorce, etc.

After marriage or divorce is finalized, or after divorce negotiations or divorce mediation have started, please submit 1 or 2 within the same month.

In the case of marriage, please submit documents that can confirm the spouse's need for childcare.

 

Note: In cases of divorce, divorce negotiations, or divorce mediation, it is necessary that the address on the Certificate of Residence be different.

Request for Transfer

The submission deadline and attached documents are the same as for new applications. (For details, refer to the cover and pages 9-10 of the "Childcare Center Usage Guide".)

 

Note: For applications in the same fiscal year, documents that have already been submitted and have no changes can be omitted.

Note: Even if you are currently on prenatal/postnatal leave or childcare leave and do not plan to return to work during the month you start using the service, you can only apply for transfer if your child is continuously enrolled at the licensed childcare center, certified child center (nursery school function part), or community-based childcare service from the time of application until the end of the month before the start month of use.

However, if you do not plan to return to work within the month you start using the service, the evaluation will be conducted with the minimum score (10 points).

 

[Example]

If you leave the above Licensed Childcare Center at the end of May, you are eligible to apply in June, but applications from July onward are not eligible.

Request for Discharge

Childcare Center Withdrawal Notification

Please submit by the 10th of the month you wish to leave.

 

Note: Those who are leaving the Home Daycare Provider should submit to the Home Daycare Provider.

Request for Closure

No documents to submit

 

Note: Please be sure to contact the facility where you are enrolled.

Note: If the closure period exceeds 2 months (3 months in the case of returning home for childbirth), please contact us in advance.

No document submission is required.

The possible suspension period is as follows.

  • Returning Home for Childbirth

Up to 3 months (If the medical institution specifies the return period and it exceeds 3 months, it will be judged individually.)

  • Reasons other than the above

Up to 2 months (If you take a break for more than 2 months, you will be discharged and need to reapply.)

Transfer Out of the City

Childcare Center Withdrawal Notification

Please submit by the 10th of the month you are moving out.

 

Note: Please submit a withdrawal notification even if you continue to attend a Childcare Center within Inagi City after moving out. Also, procedures are required to continue attending at the destination municipality.

Note: In addition to the above, notification to the facility where you are enrolled may be required. For details, please contact the facility.

About the Current Situation Survey

In Inagi City, a current status survey (once a year) is conducted for enrolled children to confirm the necessity of childcare.

If submission is not made within the deadline, the need for childcare cannot be confirmed, and the child will be withdrawn from the Childcare Center.
