Note: For those who are not first-timers, applications are limited to joining together with first-timers only -

- Please fill in the required information on the application form and send it by mail or bring it to City Hall or the Community General Support Center below.
Application forms are distributed at the City Hall Senior Welfare Division, Community General Support Centers (Hirao, Yanokuchi, Elegant Momura, Koyodai), Hirao and Wakabadai Branch Offices, and the SHIROYAMA EXPERIENCE STUDY INSTITUE.
