Eligibility

Persons aged 65 and over who have not received Long-Term Care Insurance certification and are, in principle, first-time applicants
Note: This is only applicable for those who are not first-timers when applying together with a first-timer.

Please fill out the required information on the application form and send it by mail to City Hall or bring it to City Hall or the Community General Support Center.
Application forms are distributed at the City Hall Senior Welfare Division, Welfare Center, Community General Support Center (Hirao, Yanokuchi, Elegant Momura, Koyodai), Hirao and Wakabadai Branch Offices, and SHIROYAMA EXPERIENCE STUDY INSTITUE.
