QuestionMy child moved their residence near the school to attend a school in another prefecture (or city). What will happen to the My Number Health Insurance Card and the Qualification Confirmation Certificate?

If the living expenses at the new address are supported by parental remittances and the household is considered the same, you can continue to be an insured person under the National Health Insurance of Inagi City through prior procedures, and you can use the My Number Health Insurance Card and Qualification Confirmation Certificate.

Required Items for Procedures

  • Certificate of Residence for the New Address
  • Certificate of Enrollment
  • Identification documents for those coming to complete procedures (driver's license, passport, My Number Card, etc.)
