Health Checkup, comprehensive medical examination, preventive vaccinations, orthodontics, normal pregnancy and childbirth, mild body odor or blemishes, cosmetic surgery, pregnancy termination for economic reasons, and other conditions not considered illnesses, as well as work-related illnesses or injuries covered by workers' compensation insurance, cannot use the My Number Health Insurance Card or Qualification Confirmation Certificate. In addition, illnesses or injuries caused by criminal acts, intentional accidents, fights, or drunkenness also cannot use the My Number Health Insurance Card or Qualification Confirmation Certificate.
Please be sure to contact the City Hall Insurance and Pension Division in advance if you use the My Number Health Insurance Card or Qualification Confirmation Document due to injuries or illnesses caused by a third party (the other party) such as a traffic accident.
