QuestionPlease tell me the procedure if I lose the National Health Insurance qualification confirmation document or qualification information notice.

You can complete the reissuance procedure at City Hall or the Branch Office (Hirao, Wakabadai). Please bring identification documents (driver's license, passport, My Number Card, etc.).
If a person from a different household comes to complete the procedure, a power of attorney is required.
