By applying for and receiving the "Certificate of Specific Disease Medical Treatment," and presenting it together with your My Number Health Insurance Card or Qualification Confirmation Document at the medical institution's reception, you can reduce the burden of medical expenses related to dialysis.

Required Items for Application

  • Doctor's medical certificate (or a Specific Disease Medical Care Certificate issued outside of National Health Insurance)
  • Any one of the My Number Health Insurance Card, Qualification Confirmation Document, or Qualification Information Notice
