Organization Registration Conditions

Organization Registration Conditions

Organizations wishing to register must meet the following conditions.

  1. It is an organization whose purpose is to engage in sports and recreation activities or other social education activities. However, this does not apply when using the meeting room at Inagi Central Park Comprehensive Gymnasium or the multipurpose room at Inagi Nagamine Verdy Field.
  2. It is an organization composed of "members" who are more than 10 people, and more than half of them reside, work, or study in the city, engaging in activities of the registered category (excluding non-participating family members, drivers, supporters, etc.).
  3. "Representative" refers to a person who is at least 20 years old and resides, works, or studies in the city.
  4. The "representative" and the "contact person" who receives communications from the sports facility must be included as "members."
  5. If "instructors" are commissioned separately from the "members," a list of commissioned instructors must be submitted. The number of commissioned "instructors" allowed is up to 3 per 10 "members," with one additional "instructor" permitted for every 5 additional "members."
  6. Organizations must include an estimated income and expenditure for approximately one year in the Application Form Supplement.
  7. When using the facilities, an adult (a person aged 20 or older) "member" or a commissioned "instructor" must always be responsible for management and guidance.
  8. A person must not be registered as a "member" in multiple organizations for the same category. Similar categories such as soccer and futsal, tennis and soft tennis are considered the same category. If listed as comprehensive sports, it covers all competitions, and if multiple categories are listed, duplicate registration in the listed categories is not allowed.
  9. Groups using the tennis courts at Minami-Tama Sports Plaza can participate in facility maintenance activities such as the application of nigari, which are conducted several times a year.
  10. Registration as a group for the purpose of activities as a corporation is not allowed. However, corporations specially recognized by the Mayor of Inagi City may be registered as groups under certain conditions.

Conditions for Registration as a Children's Group

  1. To use the facilities of Inagi Central Park Baseball Field, Wakabadai Park Multipurpose Square, and Inagi Nagamine Tokyo Verdy Field at children's rates, you must register as a children's group.
  2. Children's organizations are those in which all "members" other than the "representative" and "contact person" are elementary school students or younger, and the majority of the children reside or attend school within the city.
  3. For children's organizations, individuals who do not participate in activities may be recognized as "representatives" and "contacts."
  4. If you include a "leader" as a member in addition to the "representative" and "contact person," it will be considered an adult organization.

Other Topics

  1. When using the facilities, reception will not be conducted for anyone other than adult members or registered commissioned instructors.
  2. Facility usage fees are based on the registered "members." Even if a general (adult, junior high school age or older) registered group uses the facility primarily for children, the usage fee will be the regular facility usage fee.
  3. Even for children's organization registrations, if adults who are junior high school age or older other than "commissioned leaders" are included and use the facility, the regular usage fee will apply.
  4. The facility may verify the names, etc., of adult "members" or "commissioned leaders" at reception to confirm that there is no transfer or subleasing of facility use.
  5. Regarding the presence or absence of sports safety insurance enrollment as noted on the supplementary form, it is not mandatory but enrollment is recommended.
  6. Regarding the coaching qualifications and first aid certification listed in the commissioned instructors' roster, it is not mandatory; however, for first aid, it is desirable that at least one person, including the representative or members, holds the certification for safety management purposes.

Validity Period

Valid for 2 years from the date of certificate issuance (stated on the certificate)

The certificate issuance date will be at least 14 days after the application has been accepted without any deficiencies.

Renewal applications for the certificate are accepted from 3 months before the expiration date. (Renewal extends the validity period by 2 years)

Items to Submit at the Time of Application

  3. A list of group members and identification documents for all members residing, working, or studying within the city (see details). Note: For those under 15 years old (elementary and junior high school students), either an identification document or a consent form from a Parent or Guardian (see details) is required.
  4. A list of commissioned instructors (if there are any) and identification documents for all commissioned instructors (see details)
  5. Organization Regulations (for organizations with regulations only)

To verify the information provided in the Application Form, please have your identification documents (see details below) ready for confirmation at the reception.

Details of Identification Documents

(1) Residents within the city

A copy of an identification document issued by a national or local government agency that includes the address, name, and date of birth, or other similar documents (be sure to make a copy that clearly shows all three: address, name, and date of birth)

Examples of Acceptable Documents

Driver's License, Individual Number Card, Passport (with current address indicated), Health Insurance Card, Certificate of Residence (limited to those issued within one month from the issue date to the application date. Copies are not allowed only for the Certificate of Residence)

Examples of Unacceptable Documents

Notification Card (My Number), bills or contracts for electricity, gas, sewerage, telephone charges, copies of the Certificate of Residence, passports (without current address), postal items or delivery slip receipts

(2) Employment within the city

Identification document from (1) and a certificate of employment or work issued by the corporation of the workplace

Note: The certificate of employment or work must have the issue date indicated, be within one month of the application date, and be the original document only

Note: It must have the company seal stamped and include the address of the workplace within the city.

(3) Enrollment within the city

A copy of the student ID issued by the school you are enrolled in (make sure the copy clearly shows the address, name, date of birth, and enrollment period) or an Enrollment Certificate (must include address, name, and date of birth, be issued within one month prior to the Application Form date, and be the original document only)

(4) Consent Form

A designated form on which the organization name, full name, date of birth, age, address, contact information, Parent or Guardian's name, and date of entry are handwritten by the Parent or Guardian (the date of entry is valid if within 3 months prior to the application date)

 

Note

  • The personal information you have submitted will be properly managed in accordance with laws and regulations related to personal information.
  • If you wish to have your identification returned, please check the "ID Return Request" box in the member roster and the entrusted instructor roster. If marked "None" or left blank, the ID will be discarded at Inagi Nagamine Verdy Field.

  • The application for registration and the receipt of the registration certificate can be done by the representative of the organization, the contact person, and the members of the organization, including the family members living together. Only those who are equivalent to high school students or above are eligible. A form of identification will be required at the time of receipt.
  • If you wish to have your identification documents returned at the time of application, acceptance will only be at Inagi Nagamine Tokyo Verdy Field. (Verification will take some time. Also, if there are many members, we may keep the documents temporarily.)
  • Confirmation regarding the content will not be conducted except for the Inagi Nagamine Verdy Field. If there are any discrepancies in the content, we will contact the person or representative from the Inagi Nagamine Verdy Field (042-401-3960).
  • In order to review the Application Form and attached documents, it takes 14 days from the date the application is accepted to issue the registration certificate. (Note: However, if there are any deficiencies in the content, it will be 14 days or more from the date the corrections are completed. Also, if the period includes the year-end/New Year holidays or long holidays, it may take more than 14 days.)
  • Registration certificates not collected within one month from the issuance date will become invalid. (No contact will be made from the facility)
