Stillbirth Notification

Period

Within 7 days from the stillbirth date
Caution: In the case of stillbirth before 4 months of pregnancy, there is no need for Notification.

• Municipal Office of the City or Ward where stillbirth occurred
• Municipal Office of the City, Ward, or Town of the Notifier's Location

• Father or Mother
• Legal Representative
• Co-habitant
• Doctor
• Midwife
• Other Witnesses

Stillbirth Notification Form (Certification by a doctor or midwife is required)

Available Procedure Dates

Open on weekdays from 8:30 AM to 5:00 PM

• Note: On holidays when City Hall is open, the hours are from 8:30 a.m. to 11:45 a.m. and from 1:00 p.m. to 5:00 p.m.
  • Outside of the above hours, services are accepted at the City Hall night duty room.
  • On holidays and when accepted at the City Hall's night duty room, only "custody" will be available.
• Note: Notifications related to family registers are not accepted at the Branch Office.
