The Divorce Notification Form Will Change (From April 1, 2026)

Due to the revision of the Civil Code, from April 1, 2026, it will be possible to have custody of minor children after divorce either by one parent alone (sole custody) or by both parents jointly (joint custody). Accordingly, the format of the divorce notification form will also be changed.

When Submitting a Divorce Notification Using the Old Form

If you have a minor child and submit the notification on or after April 1, 2026, attaching the "separate sheet" is required.

If you do not have a minor child, you can still submit the notification using only the old form of the divorce notification even after April 1, 2026.

Note: If the checkbox for "Understanding the meaning of exercising joint custody or sole custody after divorce and agreeing based on true intent" is not checked, you may be asked to come to the office in person to complete the entry.

About the New Divorce Notification Form

The new format of the divorce notification will be distributed at the City Hall Citizen Affairs Division, Hirao Branch Office, and Wakabadai Branch Office as soon as it is ready.
