Issuance in-person method (My Number Card application)

The in-person issuance method involves applying for the My Number Card at home or elsewhere using a smartphone, mail, etc., and then visiting the My Number Card Center to receive the card upon issuance.

This is a method for those who do not have a government-issued photo identification document to obtain a My Number Card.

Application Method

Please apply in person using one of the following methods.

Application using a smartphone

Please scan the QR code listed on the "Application Form for Issuance of Personal Number Card" or "Personal Number Notification" and apply on the website for issuance. If you do not have the application form, please refer to the method of obtaining the application form at the bottom of the page.

Application using the photo booth in the town

Select "Application for Individual Number Card" from the touch panel of the available photo booth, insert money for taking the photo, and scan the QR code of the "Application Form for Individual Number Card" with the barcode reader.
Follow the on-screen instructions, fill in the required information, take a photo of your face, and submit it.

How to Obtain the Application Form (For Those Who Do Not Have the Application Form)

The ID-required application form needed for applications via computers, smartphones, or photo booths in the city can be obtained by the following methods.

Request in Person

Please bring identification documents (such as a driver's license or Health Insurance Card qualification confirmation) and request at the My Number Card Center by yourself or a household member.

Online Billing

Will be sent by regular mail that does not require forwarding to the registered residence.

How to Receive

The My Number Card must be received by the individual in principle.
Note: If the individual is under 15 years old or an adult ward, both the legal representative and the individual must visit together.

About Receiving by Proxy

• Only if the individual is hospitalized or has a physical disability or other unavoidable reasons making it difficult to visit in person, can the receipt of the card be delegated to a proxy. Please check the requirements for proxy receipt at the link below.
• Please understand that work or being busy does not constitute an unavoidable reason.
• When receiving by proxy, in addition to proof that the individual cannot come in person, such as a certificate of hospitalization or facility admission, photo ID documents of both the individual and the proxy are required. Since the required documents differ from the usual ones, please consult us in advance if you have any questions.
