The My Number Card must be received by the individual in principle. If the individual is receiving it, please see the link below.
Only when the individual is hospitalized, has a physical disability, or has other unavoidable reasons making it difficult to visit in person, can they authorize a proxy to receive the card.
When receiving by proxy, in addition to proof of hospitalization or proof of admission to a facility or other evidence that the individual cannot visit, photo ID documents of both the individual and the proxy are required.
The required documents are different from usual, so please consult in advance if you have any questions.

Medical certificate, hospitalization medical plan, receipt confirming hospitalization (issued within the last month), photo identification (created by the hospital director)

Pregnant Women Mother and child health handbook, receipt or examination ticket indicating that prenatal checkups have been conducted
Those studying abroad Copy of the visa (where the visa can be confirmed), copy of the student ID from the study abroad institution
Those on long-term business trips
(Seafarers on long voyages, etc.)
Documents that indicate long-term business trips or long-term voyages created by the employer
(Those recognized as difficult to visit the office in light of objective circumstances such as job description, work location, and work style)
Individuals who have avoided social participation and have remained at home for a long time
(For those who are withdrawn)
Photo ID (created by the public support agency you are consulting with and the head of that agency)
  • Note: The photo ID certificate serves as a substitute for identity verification documents or proof of inability to visit in person for those certified as requiring long-term care or care support, facility residents, long-term hospitalized patients, and individuals who are socially withdrawn. Please download and create it.
  • Note: Reasons such as work or being busy are not considered valid reasons, so please understand.

How to Receive

  1. When the card arrives at the My Number Card Center and is ready to be handed over, the city will send a "Notice of My Number Card Collection."
  2. After viewing the announcement, please prepare the necessary documents and make a reservation via the internet or by phone.
  3. On the reserved date and time, please bring all the necessary documents to the My Number Card Center, and have a representative visit.
    For details, please refer to the notification sent by the city.
    Note: If there are any deficiencies or omissions in the documents brought, the issuance of the My Number Card cannot be done.

Required Documents [For First-Time Card Applicants]

  1. My Number Card Issuance Notification and Response Form (White postcard sent from the city)
  2. Personal identification documents A2 points or A1 point + B1 point or B3 points (1 point must include a photo) (see below for "List of Identification Documents")

Note: If the individual is 0 years old at the time of receipt, the individual's identification document is also acceptable as B2 point.

  1. Identification documents of the agent A2 points or A1 point + B1 point (see below for "List of Identification Documents")
  2. Certificate for those unable to visit in person (see above "Requirements for Agent Pickup and Certificate for Those Unable to Visit")
  3. Notification Card (for those who have one)

If the person is under 15 years old or a ward of an adult guardian, the following documents are required.

  1. If the person is under 15 years old, a copy of the family register (not required if the person and their legal representative are in the same household and the person's registered domicile is within Inagi City) is not required.

Fee

Free
However, a fee of 1,000 yen (800 yen if the electronic certificate is not installed) will be charged in the following cases.

  • Reissuance due to loss, damage, contamination, or destruction of the My Number Card
  • Reissuance due to failure to change the address on the card within the prescribed period
  • Reissuance of My Number Card after voluntary return
  • Reissuance after the Certificate of Residence has been deleted due to reasons such as not residing
  • [Foreigners] Reissuance due to expiration of My Number Card for mid- to long-term residents (excluding those with Highly Skilled Professional No. 2 or Permanent Resident status)

List of Identity Verification Documents

  • Identification documents are limited to originals that are within the validity period.
  • Identification documents are limited to those whose "name and date of birth" or "name and address" match the Certificate of Residence.

A government-issued photo ID

  • Driver's License
  • My Number Card (with photo)
  • Passport
  • Disability Certificate
  • Driving History Certificate (limited to those issued on or after April 1, 2012)
  • Residence Card (with photo)
  • Special Permanent Resident Certificate (with photo) etc.

  • Pension Book or Basic Pension Number Notification (including Pension Amount Revision Notice and Pension Payment Notice)

  • Maternal and Child Health Handbook

Note: Digital student ID and digital employee ID cannot be screenshotted. The app must be launched and operated in the presence of staff.

Note: The photo ID certificate serves as an alternative to identity verification documents for those under 18 years old, adult wards, individuals certified as requiring nursing care or support, facility residents, long-term hospitalized patients, and individuals who are withdrawn. It also serves as proof of inability to visit in person (except when the individual is a high school or technical college student). Please download and create it from the above.

Notes

  • When issuing the My Number Card, the face photo on the card will be matched with your face. Please note that you will be required to remove masks or similar items during the face verification.
  • The notification card will be collected at the time of issuance.
  • It cannot be issued at the main office or Branch Office.
  • You will set your PIN at the counter using the touch panel.

Period from Application to Receipt

The period from application to receipt of the My Number Card is approximately 1 month (as a guideline).
If you have not received a notification after more than two months since your application, please consult us here to check the issuance status.
