The My Number Card usually takes about one month from application to receipt, but starting from December 2, 2024, it will be possible to issue the My Number Card in about one week for those who are 0 years old or have lost their card.
(1) Apply for the card at the My Number Card Center → (2) Receive the card at home (registered residence) or other locations
 

Children aged 0 can apply at the City Hall’s Family Register Section simultaneously with the birth registration. For details, please check the link below.

In the case of express issuance, a reissuance fee due to loss or other reasons will be 2,000 yen (if the electronic certificate is not included, it will be 1,800 yen).
Note: The regular reissuance fee is 1,000 yen (800 yen if no electronic certificate is included).

Requirements for Special Issuance

The issuance of express tickets is applicable to those who meet the criteria in the table below.

Note: You can apply at the same time as the birth registration

However, in the following cases, it may take more than a week for the card to arrive.

  • For those with external characters in their name or address, and where automatic character conversion is not possible
  • In case the express issuance application is crowded nationwide
  • In case of holidays and the year-end and New Year period
  • For children aged 0 years old who are not registered as residents in Inagi City

Application Method [For First-Time Card Applicants]

Please bring the required documents to the My Number Card Center and apply in person.

Required Documents

  1. Identification documents A2 points or A1 point + B1 point or S1 point + B2 points (see below for "List of Identification Documents")
  2. Notification Card (for those who have one)

Note: The photo will be taken using a dedicated terminal at the My Number Card Center (for those aged 1 year and older). If you bring a photo, please prepare one "taken within the last 6 months, with a plain background and no hat (vertical L size recommended)." Please note that photos are not required for infants under 1 year old, as their My Number Card photo is omitted.

If the person is under 15 years old or a person under guardianship, the following documents are also required.
Also, both the applicant and the legal representative must visit in person.

[Procedure for Inquiry and Response]

  1. If a request is made at the My Number Card Center, we will send the inquiry and response document to the registered residence by non-forwardable mail.
  2. Please fill in the required information on the received inquiry and response form, and prepare any other necessary documents.
  3. Within the validity period of the inquiry and response document, please bring the inquiry and response document along with the necessary documents and apply for expedited issuance at the My Number Card Center.

How to Receive

It will be sent directly by "express and simple registered mail" from national agencies to the address registered for residents.
You will receive the envelope containing your My Number Card directly from the postal delivery person.

Notes

  • Please be aware that photos taken with dedicated terminals have lower image quality than those taken at photo studios, as they are of a resolution equivalent to that of a smartphone.
  • We cannot provide the photos taken.
  • The fee for the facial photograph is free.
  • The application review is conducted by a national agency. If there are any issues with the facial photograph or other details, we will contact you by phone or other means. In such cases, you will need to visit the office again.
  • The PIN for the My Number Card will be filled out in the Application Form.
  • Those who incur an issuance fee will need to pay the fee at the time of card application. Please note that the fee is non-refundable after the application is accepted.
  • Please be sure to receive your My Number Card within the storage period at the post office.
  • If you are unable to receive your card within the storage period at the post office, the card will be returned to the My Number Card Center. To receive it, you will need to visit the My Number Card Center in person. It will be stored for a certain period, but if the storage period expires, it will be discarded. Please be aware that a fee will be charged when obtaining the card thereafter.
